Examination forms

This form is for students to complete if they have included their own publications in their thesis and these publications have co-authors.

If you have more than six co-authored publications please use the following Additional Co-authorship form for each additional publication.  The additional form/s can be added to your approval form to create one document by creating a PDf portfolio.

Create PDF Portfolios

1. Open Adobe Acrobat and choose File > Create > PDF Portfolio.

2. Drag files into the Create PDF Portfolio box. Alternatively, choose Add files from the Add files menu. Continue to add the files you want to combine. 

3. Click Create to add the files to the PDF Portfolio.

4. Save 

Students are encouraged to use the following template as the title page of their thesis.

This form is to be used by students who receive examination results which require either minor or major amendments to be made to their thesis.  This form and your thesis with tracked changes should be uploaded to Inspire for approval.

Research higher degree students can claim up to $500 towards the cost of thesis editing and optional printing and binding of the final thesis copy. For reimbursement please complete an online request through Inspire. Claims must be made within three months of completing your degree.
