Implications for engagement | Young people’s mental health is a priority area in Australia. Increasingly, mobile health interventions are being used to support and increase young people’s wellbeing, given that they are considered natural users of technology and apps. However, the potential of wellbeing apps seems to be limited. Part of the problem preventing wellbeing apps from being effective is poor engagement. This thesis focused on understanding how young Australians engage with readily accessible mobile apps to increase levels of wellbeing. Findings of this research make the case that user engagement with apps hinges on identity; as engagement requires personal investment, motivation, interest and ultimately meaning as opposed to only persuasive design features or technical capabilities imbedded in the apps. Focusing on understanding the individual first is essential to cover the gap between the app and its’ features, versus effective behaviour change and consequent increased wellbeing. |
Clare Helen Beard | Associate Professor Annabelle Wilson & Monica Cations | Homelessness as a barrier to cognitive wellbeing and dementia risk reduction | This thesis explores how the experience of homelessness impedes cognitive wellbeing and increases modifiable risk factors for dementia. It reveals multifaceted pathways that link homelessness to increased risk of dementia through a range of adverse health, social and environmental factors exacerbating the cognitive vulnerabilities of this population. The mixed methods approach shows that the absence of stable housing obstructs cognitive wellbeing and dementia risk reduction strategies. It further demonstrates that overcoming cognitive health barriers in homelessness requires inclusive, multifunctional strategies, emphasising housing stability and improved access to health, social support and mental health services. |
Lucy Charlotte Carpenter | Professor Geraint Rogers & Steve Wesselingh | Characterising the unintended consequences of antibiotic use on the microbiome and resistome: a metagenomic approach | Antibiotics are an essential component of healthcare but their increasing use has accelerated the rate of antibiotic resistance. In clinical practice, the consequences of antibiotic use such as carriage and transmission of antibiotic resistance genes in bacteria not actively causing infection, as well as disruption of the protective commensal microbiome, are rarely considered. Children and elderly people who typically exhibit microbiome instability and less robust immune responses are at high risk of poor clinical outcomes when the effectiveness of antibiotic treatment is reduced. This candidature utilised metagenomic sequencing-based approaches in two clinical cohorts of children and elderly people to characterise the unintended consequences of antibiotics on the gut microbiome and resistome. The analysis reported in this thesis suggests that the use of metagenomic sequencing should be more readily utilised in medical decision making, particularly in the development of antibiotic stewardship policies. |
David Hansman | Professor David Gordon & Santhosh Daniel | Antimicrobial Drug Resistance in Streptococcus pneumoniae | The studies described here include the first reports of antibiotic resistance in the pneumococcus Streptococcus pneumoniae. The development of resistance was important clinically because of the high prevalence of pneumococcal infections, especially pneumococcal pneumonia, which, in low and middle income countries, was often fatal in both children and adults. When my studies began, in the 1960’s, several antibiotics were available for the treatment of pneumococcal and streptococcal infections: tetracyclines, chloramphenicol, erythromycin and lincomycin, as well as penicillin. Resistance to these agents was unknown, so pneumococcal infections could be treated with these antibiotics. This work describes for the first time the detection of antimicrobial resistance to penicillin and other antibiotics in pneumococcus. This had major implications for management of serious pneumococcal infections which remain an important cause of human disease. |
Andrew Hayles | Krasimir Vasilev & Vi Khanh Truong | Hydrothermally Etched Titanium for the Mitigation of Implant Associated Infection | The rate of medical implant-associated infection has stubbornly remained static despite improvements in surgical techniques and patient care. Further complicating matters, when pathogenic microbes attach to an implant surface, they become substantially more drug tolerant, leading to a failure of antibiotic treatment to clear the infection. Due to this, the most attractive anti-infective strategy is prevention of bacterial colonization. Recent research has focused on spiked, nanostructured surface modifications which kill bacteria on contact. Hydrothermal etching of titanium (HTE-Ti) is one cost-effective and simple method to fabricate such surface modifications. In the present thesis, my research is focused on the interactions between pathogens and HTE-Ti, with the aim to assess the capacity of this surface modification to mitigate implant-associated infection. The chapters within this thesis are based on publications accepted during my candidature. |
Laurent Alexander Marijn Hendrickx | Professor Ruurd Jaarsma & Job Doornberg | Cotutelle: Intramedullary Naililng for Tibial Shaft Fractures: Pearls and Pitfalls and Complications | Tibial shaft fractures are frequently occurring traumatic injuries (16.9-21.5 cases per 100.000 per year) often affecting the young and active population. When these fractures are not treated adequately, they can result in chronic disability. Intramedullary nailing is widely considered the primary operative treatment for tibia shaft fractures but even though the procedure has been in existence for several decades opportunities for further improvement remain. Especially in our era of “personalized” medicine and data driven care, it has become more important to differentiate between what is beneficial to the population and what is beneficial to the individual patient. Therefore, the aim of this PhD Thesis was to contribute to individualizing the management of patients treated with intramedullary nailing for tibial shaft fractures in order to improve patient care. |
Andrea Natalie Natsky | Associate Professor Billingsley Kaambwa & Andrew Vakulin | Positioning Primary Care at the Centre of Sleep Health Management: A Health Economics Approach | Sleep disorders are becoming increasingly prevalent and associated with significant economic impacts. The principal contribution of this thesis is its application of health economic approaches in addressing current issues in managing sleep disorders. Five independent studies were conducted that provide economic evaluation evidence of novel strategies for insomnia and OSA, empirical preferences of patients and high-risk populations of OSA and mapping function to predict utilities from a sleep-specific instrument. The findings help inform policymakers of the cost-effectiveness of treatment and diagnostic tools for sleep disorders and optimise the development of the OSA care pathway. The thesis supports the extensive research effort in positioning primary care at the centre of sleep health management. Given the limited number of economic studies on sleep health, the findings and various methodologies used in this thesis highlight the potential and usefulness of health economic approaches in advancing pertaining issues in sleep health. |
Tarandeep Oberai | Professor Ruurd Jaarsma & Maria Crotty | Delirium in patients with hip fractures | Hip fractures are the most severe type of fracture posing an important risk for in-hospital complications and burden on quality of life. The mortality rate in the first year following a hip fracture is 25% in older adults. Delirium is one of the most common post-operative complication experienced by patients with hip fractures. This aim of this research was to improve care of this patient population by reducing incidence of delirium. The thesis includes 10 papers, published between 2018 and 2021, reporting data collected from patients as well as clinicians. The data reports on clinicians perceptions and knowledge. Additionally, patient data reports on outcomes following a hip fracture and effect of the intervention on incidence of delirium. The work is original, significant and relevant given the unprecedented global numbers of older people who sustain hip fracture. |
Nyoli Anita Valentine | Professor Lambert Schuwirth & Michael Shanahan | What makes assessment fair? | In the past, assessment of health professionals mostly focused on clinical knowledge, at the expense of skills such as communication. This was because only ‘objective’ assessment was thought to be fair. However, the health professions community has questioned whether this idea of fairness really works. This research approaches fairness from a new perspective. This thesis includes 5 papers, reporting data from different stakeholders. It identified four key elements to fairness: transparency, fitness for purpose, accountability and credibility which consistently emerged across the data behaving like a fractal within a complex adaptive system. A complexity perspective allows us to think differently about how we improve assessment. It challenges the idea that fairness is a checklist of individual components and instead it emphasises the dynamic interactions between the elements. Complexity makes assessment meaningful and fair in messy clinical realities and embraces a more holistic view of what makes clinicians safe to practice. |
Sina Vatandoust | Professor Ross McKinnon & Michael Michael | Locally Advanced Rectal Cancer: Characterising Patient-Reported Outcomes and Biomarkers in the Era of Non-Operative Management; | Rectal cancer treatment often involves chemoradiotherapy, with some patients showing complete responses allowing a non-operative 'watch-and-wait' approach. This doctoral thesis addressed gaps in knowledge about patient-reported outcomes and fear of cancer recurrence and explored biomarkers in this field. The first section studied patient-reported outcomes, revealing patients with a watch-and-wait approach showed better quality of life and bowel function compared to surgical patients. The second section, using a large dataset, suggested this approach did not increase fear of cancer recurrence. The third section conducted pilot studies on novel biomarkers such as non-coding RNAs, tumour infiltrating lymphocytes, and gut microbiota for predicting treatment responses. A correlation was identified between biomarker piR-1920878 and response to chemoradiation, and a trend towards a diverse gut microbiota in patients with improved bowel function. Ultimately, the study found a watch-and-wait approach improves quality of life without increasing fear of cancer recurrence, and suggested further research into biomarkers. |

Jennifer Lynn Richards | Associate Professor Lorna Hallahan & Hossein Esmaeili Khabr | Embodied Justice: An Integrated Faith-Law Response for Christian Women Experiencing Domestic and Family Violence in Australia | This research theorises concepts of justice for Christian women who experience domestic and family violence (DFV). Collaboration between religious leaders and domestic violence workers is now seen as best practice in addressing the multilayered needs and safety concerns involved. This thesis brings questions of justice and the criminal justice system visibly within that collaboration at a conceptual and practical level. It develops a framework for church leaders and women to use when responding to DFV which takes both legal and faith considerations into account. It investigates potential barriers to engagement with the criminal justice system caused by faith beliefs. These are addressed through identifying elements of congruence between legal and theological meanings of justice. This overcomes any perceived inconsistency between a 'sacred' and 'secular' response to violence, allowing for an integrated, faith-law response with significant restorative potential. |
